The body of the certificate, including the date, is completed in an unknown hand; autograph signatures of supporters.

MS annotation in ink: 'TRNS GARRISON W. H.'

A file card headed 'Duplicate' , with spaces for 'Name', in which is entered ‘Garrison, Wm. Herbert', and 'Proposed as' with the entry 'Jones, W. H. | 1899' is filed under 'Garrison'; there is an annotation 'Died 1935' in the same hand as completed the details. 1899 presumably refers to the date of the name-change. There is a pencil annotation in another hand '[See ref only]'.
